APPLICATION FORM FOR HOSTEL ACCOMMODATION AT MUET JAMSHORO CAMPUS

Application must be accompanied with:

i. Two (02) recent passport size photographs of the Applicant (Attested)
ii. Two (02) photocopies of Applicant’s NIC/Domicile/PRC/B Form card
iii. Two (02) photocopies of Applicant’s Father/Guardian’s National I.D. Card
iv. Declaration form Parent and Sureties/Guardian’s and sureties (in Form MUH-2) duly attested by Juditional Magistrate/D.D.O of the area of residence of the Applicant
v. Declaration Form (MUH-2) in respect of Nominee of the Government etc. Shall be signed by an authorized officer of the Nomination Agency.
vi. In case of foreign students, the recommendation and undertaking the prescribed proforma for the admission and the guarantee for the payment of hostel dues and good behavior should come from the Embassy/Consulate.
vii. Copy of Admission letter.
